If you are already at high risk for getting type 2 diabetes, antidepressant drugs can boost that risk, according to new research reported Saturday. The report was based on a re-analysis of part of the Diabetes Prevention Program, a large-scale study in which researchers reported in 2002 that those at high risk for getting type 2 diabetes who lost excess weight and exercised were able to prevent the onset of diabetes much of the time. In the new study, to be presented at the annual meeting of the American Diabetes Association (ADA) in Washington, D.C., researchers looked at the effect of antidepressant drugs on a person's proclivity for getting diabetes. They evaluated the effect on those in the three groups studied in the Diabetes Prevention Program: the lifestyle intervention group, the placebo group and the group given the diabetes drug metformin (Glucophage).
BOSTON, July 20 -- Two common genetic variants appear to contribute to the development of type 2 diabetes for some patients with impaired glucose tolerance, found researchers here. But lifestyle moves seem to be able to combat the enhanced risk. The genetic finding helps to elucidate diabetes risk mechanisms, but it may not have an immediate therapeutic payoff, reported investigators with the Diabetes Prevention Program in the July 20 issue of the New England Journal of Medicine. The authors of an accompanying editorial agreed. "Our data, combined with previous longitudinal studies and genetic findings, show that type 2 diabetes can be triggered by decreased insulin production and not just by insulin resistance," said Jose Florez, M.D., Ph.D., of Massachusetts General Hospital here.

The finding, published in the July 20, 2006, issue of the New England Journal of Medicine, follows the discovery by deCode Genetics that a variant in a gene called TCF7L2 predisposes people to type 2 diabetes. .

"The DPP is an outstanding example of a robust clinical trial that continues to answer crucial questions about the etiology and prevention of type 2 diabetes, a complex disease that is steadily rising throughout the nation and the world," said National Institutes of Health (NIH) Director Elias A. Zerhouni, M.D. The NIH, which sponsored the DPP, continues to fund the follow-up study of DPP participants.
Washington: A new study has confirmed that a gene linked to type 2 diabetes ups the risk of developing the disease, but a person can greatly reduce that risk through diet and lifestyle change. The conclusion came from the Diabetes Prevention Program (DPP), a large study of 3,548 adults at risk for developing type 2 diabetes. Researchers found one copy of the risk variant in 40 percent of DPP participants and two copies in 10 percent. They say for those who inherited two copies, the risk of developing diabetes is about 80-percent higher than it is for non-carriers. The characteristics of type 2 diabetes are insulin resistance and a gradual failure of beta cells to produce enough insulin. The study reveals this variant of TCF7L2 is linked with decreased insulin production but not with any increase in insulin resistance.
Carbohydrates with a low glycemic index may help increase weight loss, decrease body fat and reduce the risk of cardiovascular disease.
The question: Obesity remains a growing problem among adults in the United States. The condition, which is defined as having a body mass index greater than 30, increases the risk for several health problems such as hypertension, type 2 diabetes and heart disease. Physical activity and a healthy diet play important roles in prevention. Studies have found that diets high in carbohydrates and low in fat are best for weight loss, though some experts argue that the glycemic index of a food how quickly and how high it raises blood sugars must also be considered. The theory behind low glycemic index diets holds that rapidly digested, high glycemic index carbohydrates cause fluctuations in blood glucose (sugar) and insulin levels, contributing to hunger and preventing the breakdown of fat.
The number of Americans with diabetes nears 21 million. Diabetes has two main types with some subsets: Type I, formerly called juvenile diabetes typically, but not always, develops during childhood or before age 25 and affects about 5 to 10 percent of diabetics and Type 2, which affects the other 90 to 95 percent and is frequently, though not always, linked to being overweight or obese. It used to be known as “adult-onset” diabetes, however, as childhood obesity has increased dramatically in recent years, so too has the number of children being diagnosed with Type 2 diabetes. The prevalence of the latter is “bad, it’s been getting bad, and it’s getting worse before it gets better” says Frank Vinicor, M.D., MPH, director of the division of diabetes translation at the Center for Disease Control and Prevention (CDC) in Atlanta.
